Community Link Workers are non-clinical social practitioners who provide support for issues that go beyond the scope of medical treatment.
CLW’s can support individuals around a range of social stressors that affect health and wellbeing, for example, money worries, debt, unemployment, social isolation, bereavement supports, alcohol and drug use, managing stress and living a healthier lifestyle.
Our CLW can provide support around the self-management of low mood and anxiety and assist individuals over time to access the resources needed to keep them well.

A Community Link Worker, can offer support to patients with social, emotional or needs which are non medical. This services is open to all patients registered with the practice and is confidential.
